About the Role

We are seeking a caring and compassionate 12-hour Nurse to join our team at Autumn Lake Healthcare at Alice Manor, a sub-acute rehabilitation and long-term care facility located in Baltimore, Maryland. This role offers the opportunity to work in a dynamic and supportive environment with a team of dedicated healthcare professionals.

You will collaborate with occupational, physical, and speech therapists to provide comprehensive rehabilitation services and have access to professional development and continuing education opportunities to advance your skills and knowledge. Our facility values patient-centered care and prioritizes resident satisfaction.

Responsibilities

• Provide direct patient care to residents, including administering medications, performing vital signs, and assisting with activities of daily living (ADLs).
• Implement care plans and collaborate with interdisciplinary teams to ensure comprehensive care.
• Conduct routine assessments to identify changes in resident condition and report findings to the nursing team.
• Develop and maintain strong relationships with residents, families, and other healthcare professionals.
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction.
• Maintain accurate and up-to-date records of patient care and treatment.

Schedule

Now hiring for 12-hour shifts: 7:00 AM – 7:00 PM and 7:00 PM – 7:00 AM.
