Qualifications

• DDS/DMD degree from accredited dental school or residency program
• Active Georgia dental license
• Active Federal DEA and applicable certifications
• 3 or more years professional clinical experience
• Must qualify as an Independent Contractor including: maintain own malpractice insurance policy; complete W-9 for own tax purposes
