About the role

As a Private Duty Nurse (PDN) Licensed Practical Nurse (LPN), you’ll provide 1:1 skilled nursing care in a client’s home—supporting longer, consistent shifts and building relationships with the client and family. As part of Interim HealthCare, you’ll deliver comfort, safety, and dignity through individualized care that helps clients remain at home. Responsibilities

• Provide 1:1 in-home skilled nursing care per the provider’s plan of care and your nursing scope of practice, supporting extended shifts/continuous care as assigned.
• Partner with the client, family/caregivers, and care team to support goals of care and continuity across shifts.
• Assess, monitor, and document patient condition and report any changes promptly (vitals, blood glucose, intake/output, respiratory status, etc.).
• Administer medications and treatments (including insulin and IV fluids as ordered), with thorough, shift-based documentation.
• Provide skilled care such as wound care/dressing changes and support with personal care as outlined in the plan of care.
